This Privacy Policy describes how RSM Diagnostics Lab (“RSM,” “we,” “us,” or “our”) handles information in connection with rsmdiagnosticslab.com and related online forms, document resources, and communications.
Information we collect
We may collect information you voluntarily provide, such as your name, practice details, email address, phone number, and message content when you request a kit, contact the lab, or submit a form. We may also receive limited technical data such as browser type, device information, and pages visited through standard website analytics or hosting logs.
RSM’s Request a Kit page includes three Google Forms. The Account Requisition Form collects practice, facility, provider, and administrative contact details for account setup, KOLIMS portal access, and initial kit coordination. The Bulk Shipment Request Form collects existing-account shipping details needed to send blank collection kit supply to a practice or facility. The Patient Direct Drop Shipment Request Form, which is limited to existing RSM accounts, collects the patient, contact, shipping, and related clinical intake details needed to send a kit and a blank Test Requisition Form (TRF) to a patient.
How we use information
We use collected information to respond to inquiries, process kit and account requests, provide clinician support, improve our website, and meet operational or legal obligations. We do not sell personal information.
Sharing of information
We may share information with service providers who help us operate our website, forms, hosting, or communications, and only as needed to perform those services. Website request forms are hosted in Google Forms within RSM’s Google Workspace environment. For the Patient Direct Drop Shipment Request Form, RSM has executed a Business Associate Agreement with Google governing Google’s processing of protected health information in that environment. We may also disclose information when required by law or to protect the rights, safety, and security of RSM, providers, patients, or the public.
Protected health information
RSM Diagnostics Lab is a CLIA-certified clinical laboratory (CLIA ID 08D2253750). To the extent RSM acts as a HIPAA covered entity or business associate, it handles protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws.
This website Privacy Policy governs general website and business-contact information, and it describes how certain request forms are used. PHI created through laboratory testing is governed by RSM's separate Notice of Privacy Practices and its clinical, account, and requisition agreements, which control in the event of any conflict with this policy.
The Account Requisition Form and Bulk Shipment Request Form are administrative forms and should not be used to transmit PHI, patient identifiers, test results, diagnosis details, or other sensitive clinical information. Do not enter that information in those forms unless RSM has expressly requested it through an approved process.
The Patient Direct Drop Shipment Request Form does contain PHI, including patient identifiers, contact information, and diagnostic information needed to ship a kit. That form is hosted within RSM's HIPAA-compliant Google Workspace environment under the Business Associate Agreement described above. Access to responses is restricted to authorized RSM and practice personnel with a legitimate need to access the information, consistent with the HIPAA minimum necessary standard. Where you are able to place the same request through RSM's KOLIMS Provider Portal, we recommend doing so. The drop shipment form does not replace the TRF returned with the specimen, RSM's Notice of Privacy Practices, or other required clinical documentation.
Data security and retention
We use reasonable administrative, technical, and physical safeguards appropriate to the nature of the information. For information handled by the laboratory, these safeguards are maintained consistent with the expectations of the HIPAA Security Rule. We retain information only as long as needed for the purposes described above, unless a longer period is required by law or legitimate business needs.

You may contact us to update contact details or ask questions about information submitted through this website. Depending on applicable law, you may also have rights to request access or deletion of certain personal information.
